Oftentimes, there is a lack of\u00a0continuity and coordination in follow-up care, and this negatively affects everyone involved in an advanced disease situation.<\/p><p>The Palliative Care Yields Cancer Wellbeing Support (Pal-Cycles) is a \u20ac4.5 million project funded by the European Commission that aims to find the optimal transition model between the hospital and home. Led by Prof Jeroen Hasselaar from Radboud University Medical Center Nijmegen (The Netherlands), this programme will facilitate patient-centred communication and continuity of care for people with advanced cancer, reducing unplanned hospital admissions and improving quality of life at the end of life.<\/p><p>Ten partners from nine European countries<span style=\"font-size: 16px; font-style: normal; font-weight: 400;\">\u00a0<\/span><span style=\"font-size: 16px;\">(The Netherlands, Germany, United Kingdom, Hungary, Poland, Romania, Portugal, Spain and Belgium) are involved in the <span style=\"font-size: 16px; font-style: normal; font-weight: 400;\">consortium partnership<\/span>.
